Application

The Application process

Prior to entry to accommodation based support services a client is offered an informal visit to review the accommodation on offer. If the client wishes to proceed with an application for supported accommodation, then their history and support requirements must be obtained from the client themselves or their worker. This is obtained by completing the Placement Assessment. These documents should always be accompanied by the referral agencies own risk assessment and care plan in the first instance.
