Support Provision

We work together

Support workers and service users will work closely together to build up a support plan showing a clear link to the areas of need identified in the needs assessment, and any risks identified on the risk management plan.

Once this has been agreed the support worker will identify any external agencies involved in order to link and build up a more in depth support plan, support can include the following:

  • Daily health and wellbeing check
  • Support to maintain relationships, and appointments
  • Support to manage risk
  • Support to claim and maintain welfare benefits
  • Budgeting and debt management
  • Support and guidance on how to use appliances
  • Support to develop and retain independence
  • Day to day guidance and support about how to maintain a tenancy
  • An assisted and guided dietary plan
  • Support to keep bedrooms and communal areas clean and tidy
  • Advising and maintaining the security of the accommodation
  • Support to resolve disputes
  • Support to arrange repairs
  • Property health and safety checks
  • Move on to further independent living
  • Emergency out of hours support where necessary
  • Support to register with social housing organisations

Meals

Provision of food, fluids and general household supplies are included within the contribution charge, those who wish to have meals prepared by staff will incur an additional charge – details of which can be provided on initial contact.
